Not sure what you are looking for but the basics:
1. Contact information - name, address, phone numbers
2. Date and times for departure and return or trip plans
3. How many in group
4. Any special needs - food, handicap, etc
5. Past experience
6. Emergency contact information - depends on the type of trip may or may not be needed
7. Payment information for deposit
Hi i have booked boats on the inland waters for about 40 years and in that time I have never been asked any questions from a tour operator. They always stress that you do not need any previous experience of boating. This has been the same for both river and canal boats including sail boats on the broads. I have however been asked when hiring sea going boats details of the experience of the crew or at least the skipper and on taking overa sea going boat we have been asked to prove experience, this has been done by showing log books and certificates.

Depends where you want to hire.
In France you need to have an International Certificate of Competence - this can be issued by the hirer for a couple of weeks if you have experience but it is probably simpler to apply to the Royal Yachting Association with proof that you have a Helmsman’s certificate and possibly even RYA Dingy qualificatons; saves a lot of hassle on foreign soil.
English Canal boat hirers are supposed to show first timers at least the rudiments of lock operation if they have had no experience so they might ask you if you have operated locks before and checked that you know a little about the “rules of the road”. Generally everything is very casual.
